Virtual folder

Here's how to reproduce that bug:

create a new project (whatever the type is), then create a new Virtual folder and add a new file to the project. The virtual folder disappear. Even if Code::Blocks is closed and reopen, the virtual folder is still missing and you can't add a new one with the same name.
